Criminal Record Checks

The Perth Police Service provides this service for residents of the Town of Perth. The only exception is for non-residents who are volunteering within the town. Agencies are required to have individuals apply separately unless there is a contracted agreement with the Perth Police Service.

 


Requirements:
To apply for a criminal record check, you must attend the Perth Police Service at 1881 Rogers Road in Perth Ontario and fill out an application. There are 2 forms for these checks. If you are going to be working or volunteering with young persons, seniors or handicapped, you will be required to fill out a Police Record Check for the Vulnerable Sector.

Volunteers are required to present an original letter on official letterhead from the local organization that intends to engage them in a volunteer capacity. The letter must have the applicant’s name on it and must be signed by the person in charge of volunteer services.

At least two pieces of identification that confirm your name, date of birth and address must be presented with the application. One piece must have a photo of the applicant. Ontario Health Cards cannot be accepted for identification purposes (Health Cards and Numbers Control Act, 1991 section 2.2(1).


For the people who do not have picture identification, you must complete an Alternative Photo Identification Form which are also available at the Perth Police office. For this form, you need to have a guarantor to sign. The Guarantor must know you for a minimum of 2 years and be present with you when you apply. You also need a passport size photo as your picture identification.

In the case of a possible match, individuals who require a copy of their criminal record will have to be fingerprinted by the Perth Police Service by appointment only. You can call 613-267-3131 for an appointment. The fee of $20.00 for this service is waived for volunteers only. Fingerprints will be forwarded by the Perth Police Service to the R.C.M.P. along with the applicant’s certified cheque or money order in the amount of $25.00 (payable to the Receiver General of Canada). Results of the search will be mailed directly to the applicant. The RCMP fee is not required if the applicant provides written confirmation of volunteer work from a bona fide registered non-profit organization.

The search includes National databases and our own local database. For applicants who have resided less than five (5) years in the Town of Perth, the police agency from their former address will be contacted for any information in their records. Police Record Checks are processed in approximately 15 working days assuming timely response from other police services. When the record check has been completed, it must be picked up in person by the applicant or by someone presenting a letter of permission from the applicant, dated and signed by the applicant whose signature must match the one on the application
.
